Preventive Health Assessment Terms of Service

Your employer/the organization you are affiliated with has entered into a services agreement with TELUS Health Care Centers Inc. (THCC) (the “Services Agreement”) and under the terms of the Services Agreement you are an “Eligible Person” under the Preventative Health Assessment Program offered by THCC (the “PHA”).

The PHA is a screening, preventive, assessment provided by a multidisciplinary team. A comprehensive summary of this assessment will be provided to you, so that you can follow up with your own family doctor.

The PHA program does not include any primary care or follow up. The PHA program is not, and should not be used as, a replacement for your family doctor.

Under the terms of the Services Agreement, your employer/the organization you are affiliated with has negotiated a preferential rate with THCC. You remain solely responsible for the payment of the service.

You acknowledge the following:

  1. The services to be included in the PHA, and all of the terms of these services, are set out in the Services Agreement;
  2. You will contact your employer/ the organization you are affiliated with should you have any questions about which services are included in the PHA; and
  3. The PHA program does not include any primary care or follow up. The PHA program is not, and should not be used as, a replacement for your family doctor.
